You cannot get SAT form from anywhere. Because, you should apply for SAT exam through online. You can apply for SAT exam by visiting the following link

Exam fee for SAT exam is $52.50(approximately 3255 rupees). After applying for the exam, you will get a conformation mail of your exam date. You should carry "printout of that Mail" to attend for the exam on that date.

The application procedure for SAT is online and you don't have to buy any hard copy of the application form. You just have to fill the online application and pay the application fee online.

Therefore, get yourself registered for SAT by visiting the official website. You can pay application fee through cheque and money order also.
