Should I refill the AIEEE form after pasting the coloured photo?

the photo in aieee form has to be black and white but I have stuck colorful photo.So should refill the form again?

you can past the coloured photograph in the form but it should be clear & covering 75% portion of your face.

i have pasted a Colour Photo without a name and date. Will my application form be accepted?? if not, then what sholud I do???
