I think you are confused about the exam

IPS, IAS IFS etc all are through a single exam i.e Civil Services Exam.

Exam is conducted in 3 stages.

Preliminary Round ( Screening exam )
mains ( only those are allowed who clear preliminary exam)
Interview ( only those who clear mains)

After clearing interview you would be designated IAS, IPS, IFS etc based on your merit or rank. You need not fill separate form for IPS.
