In the entire registration form, Part I of NDA exam, there is no such column as you have mentioned. There is no age relaxation in the NDA exam conducted by UPSC. The age limit of 16 years 6 months - 19 years is applicable for candidates belonging to all categories and there is no relaxation of age for any particular category.
The Part I of registration form comprise of the following details:

Name
Gender
Date Of Birth
Father's Name
Mother's Name
Nationality
Marital Status
Physically Challenged
Community
If you belong to Minority
Fee Remission claimed
Educational qualification
Address

my DOB is 25th JUNE 1995 and the maximum age criteria for NDA II is 2nd JULY 1994 ...............can i get any age relaxation ???...its just a matter of 7 days.......only 7 days can't destroy someone's dream
