Yes you are eligible to re appear in intermediate examination even after completing graduation.
You only need to submit all your documents.
You can pursue from any of the board. It depends on you that from which board you want to appear. You should apply as a private candidate.

No, you cannot get re admission in class 12th once after the completion of Graduation course.
You can get admission in class 11th in order to pursue class 12th.
It will be better for you to get admission in class 11th in an open School to complete your class 12th studies.
