How to fill Combined Medical Service Examination Forms?

The Combined Medical Services (CMS) Examination is held by the Union Public Service Commission (UPSC) of India. The exam is conducted to select eligible doctors to services such as Municipal Corporation of Delhi, Central Health Service, Health Service, Ordinance Factories, and Railways. The candidates can apply for the exam through either offline or online mode.

Instructions to be followed while filling the form

Following are the instructions that should be followed by the online applicants-

  • Visit http://www.upsconline.nic.in/ to download the application form along with the instructions.
  • The candidates should complete both the parts of the application following the given instructions.
  • The candidate should pay an amount of Rs. 50/- in any of the SBI branches.
  • The applicant must have his/her photograph which is duly signed and scanned in the .png format before t filling the form.

Listed below are the instructions that are required to be followed by the applicants using the offline mode.

  • The candidates have to collect the application form and the information brochure from the concerned Head Post Offices or Post Offices.
  • The candidates have to remit Rs. 100/- for the application form.
  • The candidate has to fill the form in his/her own handwriting.
  • They must use only HB pencil for darkening the circles whereas they can use black or blue pen to write in the boxes.
  • The candidates must have intense care while darkening the circles.
  • The entries in the boxes and those made by darkening the circles must be the same.
  • Make sure that the signature put by the candidates in all the places must be identical.
  • The candidates can never change the entries made in the original application after submission.
  • The candidate should select only one center for the examination.
  • The candidate must write the name of examination, email address, and the application form No. on the acknowledgement card.
  • The applicant should paste a postage stamp of Rs. 6/- on the card.
  • The applicants should ensure that the filled in application will reach the destination before the closing date.

Exemptions to be taken care while filling in form

Following are the exemptions to be taken care while completing the application-

  • The SC/ST candidates will have an age relaxation up to 5 years.
  • The candidates from other backward classes will get a maximum age relaxation of 3 years.
  • The SC/ST candidates, female candidates, and the physically handicapped candidates are exempted from remitting the fee.

Acknowledgement of exam application form

After submitting the completed application form, the candidates will get the Acknowledgement Card within 30 days. Those who did not get the card should contact the Commission by quoting the Name & Year of examination and Application Form No. Those who submit the filled in application form by themselves at the Commission Counter will get the Acknowledgement Card at the counter itself.
