NEET refers to National Eligibility cum Entrance Test and the same examination is held by CBSE once every year for offering admission to undergraduate medical degree course across any medical colleges of India excepts AIIMS and JIPMER.

Further, the minimum education required for appearing in NEET is a Class 12th in Science stream with Physics, Chemistry and Biology as main subjects with at least 50% marks in PCB group subjects from a recognized board. Marks relaxation of 10% is allowed for reserved category candidates.

So, having opted for Mathematics as an optional subject for your Class 12th academics and secured 45% marks in the same you are eligible for appearing in NEET if your aggregate in PCB group is 50% ( or 40% in case you are a reserved category candidate ).

NEET is a National Level Medical Entrance Exam.

In your optional subject you just have to pass.
Its ok if you are getting 45%,it doesn't matter for NEET.
